KDE 3.x series default theme was the plastik theme.
So check that use plastique theme and plastik window decoration.

The icon set is not availalble (AFAIK) but you can use something else for that, like Nuvola.

For Plasma-desktop theme I would suggest as well the Aya.

Recently I found out about the Trinity project, witch is a fork of KDE3 and can be downloaded at http://trinity.lv/ integrated in kubuntu, or at http://www.trinitydesktop.org/
